Dear Parents and Patient(s),
It is with mixed emotions that I inform you of my decision to retire from my practice on September 1, 2021. This was a difficult choice for me to make because of the special relationship that I have developed with each of you and your family over the years.
All patient families will be receiving a letter in the mail with more information. In the meantime, I kindly ask you to call my office at (847) 428-1179 to make an appointment for a school physical, sports physical, annual physical or check up for your child or children before August 28, 2021.

founded in 2002. Dr. Thida Maw provides excellent and
complete care for children from birth to young adulthood.
